- Review and verify patient prescriptions for accuracy and appropriateness.
- Consult with healthcare teams to provide expert advice on medication therapy.

🎓 Kualifikasi & Syarat
- Bachelor's degree in Pharmacy or a related field.
- Minimum of 3 years of experience in clinical pharmacy.
- Certification as a Clinical Pharmacist (e.g., BCPS, BCACP).
- Proficiency in using electronic health records (EHR) systems such as Epic or Cerner.
- Strong communication and interpersonal skills.
- Ability to work in a fast-paced, multidisciplinary healthcare environment.
